We developed and are testing the effectiveness of a telehealth psychological intervention called NeuroCARE to reduce caregiver anxiety symptoms, mitigating distress/burden, improving quality of life, and increasing coping in this unique population.
Assessments
Patient assessments are a critical part of the medical world, and nurses play a key role in helping patients understand their needs. They build rapport through friendly small talk, and provide a basic walkthrough of what to expect moving forward. This helps reduce the stress that patients often feel, while boosting confidence and preparing them for future steps in their health journey.
During the assessment, your healthcare team reviews your symptoms, medical history and medications. Then, they perform neuropsychological testing to assess your thinking processes and memory. This provides unique insights that can help your healthcare provider determine whether changes in your mental functioning are normal signs of aging or if they point to a neurological disorder like dementia. It also gives your healthcare team a baseline that can be used to track your progress over time. They’ll then use the results to identify your strengths and weaknesses and make recommendations for treatment and interventions services.

Consultations
People with Parkinson disease and their caregivers report mixed satisfaction with remote consultations. Similar findings to a qualitative study during the COVID-19 pandemic show that perceptions of remote interactions vary between individuals, and that services should be flexible allowing for individually optimized timing and communication methods. HCPs should also consider mental capacity for suitability to remote consultation.
Patients may feel their health care provider is less invested in the remote consultation than in-person visits. They can often experience frustration with long appointment times, lack of pauses for questions and answers, the inability to express body language or eye contact, and lack of privacy.
The use of remote consultations is increasing. One study shows that palliative care (PC) consults significantly shortened the neuro-ICU stay of a group of patients admitted for more than 24 hours. PC consults facilitated discussions of prognosis and values, identified conflict, and clarified goals of care. Case Management
This is the process of building a care plan that will assist a specific client in navigating and controlling their healthcare journey. It involves assessing needs, creating a care strategy, and monitoring the plan’s success. It also includes coordinating with different providers and agencies to ensure the patient receives proper care in a timely manner.
Nursing case management is a key component of providing accessible health services to all populations, especially those living in underserved communities. Previous studies have shown that nursing case management is effective in reducing healthcare costs and preventing readmissions and rehospitalizations.
Caregivers of patients with primary malignant brain tumours face considerable psychological distress and burden. We have developed and are testing a telehealth intervention called NeuroCARE to reduce anxiety symptoms and improve caregiver quality of life in this population. The evidence-based behavioural therapy approach is rooted in cognitive behavioural theory and incorporates mindfulness, coping effectiveness training, and assertiveness training. The programme has also been developed to minimise barriers to participation.
